Proximal Tibia Opening Wedge Osteotomy for Treatment of Genu Recurvatum after Ipsilateral Femoral Shaft Fracture



Abstract
Several surgical techniques are described for the treatment of genu recurvatum secondary to premature anterior physeal closure of the proximal tibia. This study attempts to evaluate the clinical and radiological results of the open wedge tibial
osteotomy, superior to the insertion of patellar tendon without breakage of posterior cortex. Iliac bone and artificial bone inserted in opening wedge site and two Steinmann pins were inserted. 14 patients have been treated surgically for osseous
genu
recuvatum. All of them had history of ipsilateral femoral shaft fracture from 3.2 yrs to 8 yrs ago. There was no abnomality on knee X-rays at the time of the accident. 10 patients had complained of severe pain around the knee joint after femoral
fracture. Average age of the patients was 18 (ranging from 16 to 22). All patients complained deformity, and 8 of the 14 the 14 had pain at the time of the first visit. The length of follow up were 2.5 years on an average (ranging 1 to 4 yrs).
The
results were analysed by Moroni's scoring system based on radiologic and functional results. 6 had excellent results ; 6 had 'good' results and 2 'fair' results. This procedure offers the advantage of rapid healing. Good correction of deformity,
restoration of normal tibial plateau angle and fewer complications.
